Although we generally accept plans from all major insurance carriers, we request that our patients check with their individual carriers to ensure that our providers participate with your specific plan.

FAQ

For each appointment, please bring the following documents

  • Your insurance cards
  • Your photo ID
  • Physician referral or copy of insurance referral
  • Forms
  • Co-payment
  • A list of medications you are taking, prescribed, over the counter, supplements or vitamins
  • Any recent lab or blood work results
  • Any recent X-rays or MRI results
  • Any relevant records from other doctors or hospitalizations
  • A list of questions you would like to ask the Doctor

Other helpful information:

  • Any names and contact information for person(s) you want us to release medical information to
  • Name and Phone number of the Pharmacy that you wish to use

Late & No-show Policy

We would appreciate and require a courtesy call at least 24 hours prior your scheduled appointments if you are not able to make your appointment. There will be a $50.00 fee for missing a scheduled appointment. If you are more than 15 minutes late you will have to be rescheduled.

COVID Message

Please stay home if you feel unwell, if you have a fever, cough and/or difficulty breathing or have been in close contact with a person known to have COVID-19, or you yourself have COVID. Our office is currently mask optional and we will continue to follow the CDC guidelines. The Department of Health recommends wearing a face mask whenever you have symptoms of COVID-19, tested positive, were recently exposed to someone with COVID-19, or live in a county with elevated or “high” COVID community levels. If you test positive for COVID we are following the 10-day recommendation for quarantine for you to come into our office.
